A general consultation is required prior to booking any other service(s) offered. All client intake and consent forms must be submitted in full prior to processing to confirm any appointment. A valid photo ID is required at the time of service.
Appointments allow a 15 minute grace period; if the practitioner waits beyond this, a late fee may be incurred or the appointment may be rescheduled.
Please adhere to the cancellation policy to avoid additional fees. Missed, late, or cancelled appointments outside the policy window are non-refundable.
Booked appointments are non-refundable and non-transferable. Each session is reserved exclusively for you, with practitioner time, travel, and preparation dedicated to your personalized wellness experience.
Supplements are purchased separately unless otherwise specified. Clients may select their own preferred brand while following the practitioner’s recommended dosage. Brands recommended by the practitioner are not affiliated with or sold directly by Aesthetic Vitality.
Therapies (nutraceuticals, injections, and other services) may be subject to availability based on inventory. Aesthetic Vitality does not guarantee the availability or outcomes of any therapy.
Injection therapies are provided in partnership with trusted, independent nurse practitioner providers. Clients complete the provider’s own intake and follow their policies, procedures, and protocols, which are separate from Aesthetic Vitality. Providers use their own medical equipment and staff as needed. While Aesthetic Vitality partners closely to ensure a seamless experience, we do not administer or control the treatments and are not responsible for the medical oversight of these providers.
Phlebotomy (blood draws) performed by Aesthetic Vitality is not considered an injection therapy. No therapeutic substances are administered; phlebotomy is performed solely for diagnostic or wellness assessment purposes. Clients consent to the collection, review, and use of relevant medical information, vital signs, and laboratory data as part of their wellness evaluation and personalized recommendations.
Services provided are wellness-focused and are not intended to diagnose, treat, cure, or prevent any medical condition. Clients are responsible for consulting their physician regarding medical care, diagnoses, or medication management.
In-home services include professional travel time and scheduling buffers to ensure continuity of care and service quality.
All clients must be 18 years of age or older to receive services.
